Instructions for answering the next two questions: Do not include convictions that were sealed, eradicated, erased, annulled by a court, or expunged, or convictions that resulted in referral to a diversion program. (A criminal conviction is not an automatic bar to employment. However, failure to list a felony conviction will disqualify you from employment.)

Intermediate Perform skill with basic supervision and little amount of training needed.
Basic Perform skill with constant supervision and a medium amount of training needed.
No Experience Has not performed skill or assisted another person performing skill.

I certify that all the information provided by me on my employment application is true, correct, complete, and without material omission. I understand that omissions or misrepresentations of any information provided or requested will be grounds for withdrawal of my application from further consideration for employment, or termination of my employment if the omission or misrepresentation is discovered after hire. I agree that Coonrod Electric Co. shall not be held liable in any respect if a job offer is not extended, is withdrawn or my employment is terminated because of false statements, omissions or answers made by me on this application.

I hereby authorize Coonrod Electric Co. to contact current or former employers, schools to obtain any and all information and or records Coonrod Electric Co. considers relevant to the evaluation of my application for employment. I authorize all persons, schools, employers and other organizations mentioned in this application to provide Coonrod Electric Co. with any and all information requested by it related to my qualifications for employment. Further, I voluntarily and knowingly release any person, agency, current or former employer, schools and Coonrod Electric Co. from any and all claims, damages, or liabilities whatsoever resulting from furnishing this information.

If I am employed with Coonrod Electric Co., I will comply with all rules and regulations as set forth in any communication distributed to employees. Further, I authorize representatives of Coonrod Electric Co. to furnish information regarding my employment at the company if such information is requested in the future. I release Coonrod Electric Co. from any liability for any damages whatsoever resulting from furnishing this information. I understand that if I receive a conditional offer of employment, I will be required to pass a drug test. I understand that positive findings on the drug test will result in immediate disqualification and the removal of my name for consideration for the position for which I am applying. I hereby agree to submit to such tests and release the results to Coonrod Electric Co.

In compliance with the Immigration Reform and Control Act of 1986, I understand that I will be required to provide approved documentation that verifies my right to work in the United States upon employment. I further understand and agree that, if employed, my employment is “at will” and may, regardless of the date of payment of wages or salary, be terminated for any reason and at any time without previous notice by the company or me. I further understand and acknowledge that nothing contained in Coonrod Electric Co.’s Employee Manual or other policies or procedures negates or modifies the foregoing. I acknowledge that no Company representatives may make a contrary employment agreement. Only Coonrod Electric Co.’s Vice-President can alter an individual’s at-will status and such agreement is only valid if in writing and signed by him.
